Would a middle to high ranking soldier be making orders to kill or torture?

It is for a history project and I had thought that only high ranking officers or soldiers would make calls to kill or torture, or cut people’s hands off. So, would a middle to high get orders from, for example, a king then relay the message to the rest of the army.

    If your in action no matter what rank you are and you need to get some information out of someone for whatever reason then of course you would be giving out orders to do this, if your unethical that is, but I imagine for the most part you will just be giving out orders to get prisoners sent to a prison camp and that’s it.
